We know how hard it is to break a nicotine addiction so we look for every opportunity to make the process easier for you. In support of that, we’re proud to launch our new eIntake form; an online interactive experience to help you better prepare for your appointment/quit date.
Over a decade in the making
We have spent 12 years continuously refining our unique approach to quitting smoking or using other nicotine products. It’s a bit overwhelming to take it all in at a single appointment. The eIntake was designed with this in mind. The amount of time a person plans and prepares for quitting is correlated to their success rate, so we developed the eIntake to engage our customers in this critical part of the quitting process.
One resource, many benefits
The new digital tool will help manage all the information and support provided by Alpha Lifestyle Center by breaking the program into different phases, or components. Personally developed by a Tobacco Treatment Specialist, we know another great advantage of the eIntake is for us to better understand your unique needs and experiences ahead of time. This will help us customize your appointment even more.
The eIntake form collects your contact information and information about your nicotine use and experience with quitting. It is customized with graphic animation and text based on your unique responses. This encourages personal reflection and allows you to explore topics based on your interests. If you’re dependent on nicotine in any form you will benefit from our program. This includes cigar smokers, smokeless tobacco users or individuals who vape, use eCigarettes and hookah.

In testing the new eIntake form, client feedback has been extremely positive. As a result, we are eager to communicate this program enhancement. Alpha Lifestyle Center is already an origianl, but our commitment to excellence and innovation in this field never stops.

Karla Davers of Kdavers Design created the graphic animation for the eIntake, as well as the customer video that is viewed during the appointment. I was particularly impressed with Karla’s ability to convert my stick-figure drawings into thoughtful, understandable and engaging animation. We started working with Karla in 2015 while she was a student at Baker College. She recently completed an Internship at Disney and is accepting freelance work in the field of graphic animation.
